What we do

As we get older, we can find that we slow down, and things can take longer than it did when we were young.

This is the case for us recovering from illness or injury. It can take us longer to bounce back, get back on our feet or get back to our daily routine and very often we need a little bit of help and support.

We often need some support at home that can help us build our confidence and our daily routines until we feel like ourselves again.
Our home care and reablement service is designed specifically for this issue. If you are lucky, you have been provided with reablement services from your local council or NHS provider but in most cases, this will only have lasted six weeks.

We know that this is not enough, so our team of Occupational Therapists have developed a specialist team of reablement home care workers who can provide an extension to the reablement services to help you maximise your skills and to minimise your need to rely on long term care services.

In most cases care companies are looking for long term care cases and their practice reflects this; this practice can disable people in their own homes and create a reliance on Home Care Services. Our team has an entirely different approach. Our job is to help you get back the skills that you may have lost through hospital stays, illness or injury so you can become as independent as possible.
Essentially, our team collaborates with you to actively to make themselves redundant so you can maintain your independence and not rely on council appointed carers.
We provide our reablement input in blocks of 12 weeks to provide you with enough time to make meaningful improvements. At the end of each 12 week block we will review your goals and progress and where required, develop further intervention.

You are at the heart of this service, and we work towards your goals and your routines.

Did you know that when reablement was first created in the NHS it was not focused solely on personal care tasks but was there to enable people to return to activities such as cooking, housework and even access their local community. Unfortunately, with the reduction of the service the input has been reduced down to helping you get back to washing and dressing and you have got to try to achieve this within six weeks.

An effective reablement service gives you the opportunity to sit down with a clinical professional (in our case our clinical specialists’ Occupational Therapists) to identify what activities and skills you have lost as a result of your recent illness or injury.

The clinician helps you to develop a plan and strategies to regain lost skills and activities and then you are supported by a highly skilled rehabilitation team to work on these until you have achieved your goals.
